Catastrophic diseases are still the main cause of the increase in funding for services at BPJS Health. Seven groups of high-cost diseases are recorded as consuming around 26.42 percent of the total service burden each year.
“There are around seven high-cost or catastrophic diseases, which consume around 26.42 percent per year of the burden of BPJS Health financing services,” explained the Director of Planning and Development of BPJS Health, Sutopo Patria Jati, in a press conference Wednesday (4/3/2026).
He emphasized that controlling these diseases is a priority going forward. “We will try to prioritize and control it,” said Sutopo.
However, what is most striking is the change in the disease map in the last year. If in 2024 cancer will still be the second largest contributor to costs after the heart, in 2025 its position will be shifted by kidney failure.
In 2024, heart disease will rank first with 22.55 million cases with a cost burden of IDR 19.25 trillion. Below that, cancer recorded 4.24 million cases with funding of IDR 6.48 trillion, followed by stroke with 3.89 million cases which cost IDR 5.81 trillion and kidney failure with 1.44 million cases which cost IDR 2.76 trillion.
A year later, the data changed drastically.
Even though heart disease is still the biggest budget drain with 29.73 million cases and a cost of IDR 17 trillion, the second position is now occupied by kidney failure, with a jump in cases of up to 12.68 million and costs rising to IDR 13 trillion.
Cancer fell to third place with 7.19 million cases and a cost of IDR 10.3 trillion, while stroke recorded 9.53 million cases with a cost of IDR 7.2 trillion.
Kidney Failure Increases 9-fold
The spike in kidney failure is a major highlight. Within a year, the number of cases jumped almost ninefold, while costs increased more than fourfold. This figure shows the burden of long-term therapy such as hemodialysis that patients must undergo routinely.
The trend of heart disease cases increased significantly from 22.55 million to 29.73 million, but funding actually fell from IDR 19.25 trillion to IDR 17 trillion. This could indicate service efficiency, shifts in claim patterns, or changes in financing governance.
On the other hand, cancer and stroke have both experienced increases in cases and costs. However, the increase was not as drastic as what happened in reports of kidney failure.
